transitioning from pediatric to adult doctor care

Parents of special healthcare needs children in pediatric doctor care are advised to think ahead and plan for adult doctors for their children, and to transition them as early as age 14-18, otherwise they may be delayed in finding adult care doctors for continuing a course of care.
