​Partnered with other states, the portal provides a one-stop-shop for information related to supporting children and youth with special health care needs (CYSHCN) and their families including the Utah Medical Home Portal Services Directory of service providers.

​AAHD Resource List
The American Association on Health & Disability has compiled a highly descriptive resource list of UT state health and disability programs, including these programs of the Utah Dept of Health:  Bureau of Health Promotion Arthritis, Asthma, Diabetes Prevention and Control Programs, and Division of Community and Family Health Services Children with Special Health Care Needs (CSHCN); these programs of the Utah Dept of Human Services: Division of Services for People with Disabilities and Division of Substance Abuse and Mental Health; and these programs of the Utah State Office of Rehabilitative Administration: Services for the Blind and Visually Impaired (DSBVI) and for the Deaf and Hard of Hearing.
